It sounds like you need to be very careful to get good results from that product. That was a lot of very specific directions to follow. While it may give a good replacement when done right, I don't see how it will fair any better from future troubles.
That said, I don't know why I didn't think of this suggestion when you first posted this...
Crumb rubber + resin surfacing. It is how I plan to eventually pave my entire driveway. It should never have any of the cracking or deterioration problems that you are wanting to fix because it is flexible enough to expand and contract with the freeze/thaw cycles. It is also non-slip with better traction for walking and driving, is super durable and easy to plow or shovel snow away with no damage, and is highly shock absorbent which is good for walking, extended standing, and even saving a dropped bottle of beer from shattering. It is also a permeable surface so it never collects pools of water. Oh, and it can be any color you want. It is considered "safety surfacing" and used for playgrounds, and is an eco-friendly solution.
